Fill in all mandatory information to send the self-disclosure form.
Once you have completed all the mandatory fields, you can send the self-disclosure form. If any information is missing in the mandatory fields, this will be highlighted in orange. As soon as the mandatory information has been completed, it will be highlighted in green. Please make sure that you click on Save after you have edited the questions so that they are accepted.
In the last step you must click on the "Send" button. You will then see the corresponding window.
